If patients of Rex Medical Clinic want to obtain their medical records or have the records transferred to their new clinic, they should send an email to [email protected]

The patient’s new clinic is able to make the request with the patient’s written authority.

The records will be provided to the patient or to the patient’s new clinic on a USB drive.
